Position Summary
This position will be responsible for providing services and support to the post-pandemic recovery effort for the residents within the city of Albany by actively linking them to needed government benefits, services, and/or goods. With the support of the team leader and other Wellness Advocates, we will remove barriers and achieve positive outcomes for the health and well-being of marginalized, disadvantaged, and underserved city of Albany residents by actively linking them to needed government benefits, services, and/or help them obtain essential goods. This is a short-term service delivery model with the goal of resolving referrals within six to eight weeks.
Essential Duties and Responsibilities include the following. Other duties may be assigned. Employees are expected to be awake and alert while working.

Maintain a caseload of 10 – 12 clients and meet service needs within a six to eight week time frame.

Receive referrals and collaborate with referral source, as needed.

Rapidly engage clients and complete intake paperwork including the Social Determinants of Health questionnaire.

Effectively interface with government offices, social service providers and other community resources to actively link clients to services, assist in their application for benefits, and to obtain concrete goods.

Maintain positive communication with clients and provide active assistance including transportation, attending meetings, complete applications, etc.

Advocate for client with providers, as well as troubleshoot barriers that impede or otherwise negatively impact the ability to access services/resources.

Assist clients by providing education, information and support.

Complete documentation and data collection, as needed.

Become familiar with government benefits, community and neighborhood resources, etc.

Participate in and contribute to team meetings. Attend relevant program development activities, trainings, and conferences.

Education and/or Experience
Requires a High School Diploma or equivalent at least two years of relevant experience.
Certification/Licensure
Requires a valid driver’s license.
