Clinician, Youth ACT

Licensed Social Workers or Mental Health Professionals are eligible for a $7500 signing bonus!

As a Clinician in the Youth ACT program, you will help children avoid hospitalizations and out-of-home placements by providing intensive, community-based services to these youth and families. The youth served by Youth ACT have not found success in traditional mental health and community treatment. Our Clinicians are part of a tight-knit multi-disciplinary team that meets daily to provide medication treatment, therapy, skill building, and psychoeducational support to youth and families to meet all of their needs. You will report to the Director of Youth ACT and you will work on-site and in the community traveling to client's homes required.

Responsibilities:

  • A shared caseload and ample supervision and support to provide intensive services to the 36 youth and families that the team serves overall, meeting the client where they are in the community, rather than in a clinical office setting.
  • Specialized training in the Youth ACT model as well as the principles of person-centered planning, motivational interviewing, trauma-informed care, social determinants of health, cultural competence, implicit bias, and many other best practices.
  • A lead role in providing services to clients/families such as:
    • Risk Assessment
    • Substance Abuse Screen/Assessment
    • Suicide Assessment
    • Mental Health needs assessment
    • Social Needs Screening - Health Leads
    • Comprehensive Psychosocial Assessment to include diagnosis and needs to develop a treatment plan (IAP)
    • Child and Adolescent Strengths and Needs Assessment
  • Close collaboration with collateral providers involved in the lives of the youth and family
  • Opportunities to provide holistic care to the youth and family system via individual, group, and family counseling modalities.
  • Community-based work, so flexibility in scheduling and opportunity to be out in the community rather than tied to office setting.

Requirements and Education:

  • Master's degree is needed in this position.
  • NYS professional licensure: LMSW, LCSW, LMHC, LCAT or LMFT
  • One year post-graduate experience preferred.
  • NYS driver's license as you will be community-based and travel is required.
