Job Summary

A Clinical Care Assistant within the High Touch Model will play a crucial role in providing compassionate and personalized care to patients, emphasizing a holistic approach to healthcare. This position requires a deep commitment to patient well-being, effective communication skills, and the ability to collaborate with a multidisciplinary healthcare team. The High Touch Model places emphasis on building strong patient relationships and tailoring care plans to individual needs, promoting a patient-centered and empathetic approach.

  • Patient Assessment and Monitoring - Conduct comprehensive assessments of patients' physical, emotional, and social well-being. Monitor and document patients' vital signs, symptoms, and overall health status. Regularly evaluate the effectiveness of care plans and make adjustments as needed.
  • Patient Engagement and Education - Build strong, trusting relationships with patients through open communication and active listening. Provide education to patients and their families about their health conditions, treatment plans, and self-care strategies. Foster a supportive and empowering environment to encourage patient involvement in their healthcare decisions.
  • Collaborative Care Planning - Work closely with the healthcare team to develop and implement personalized care plans. Collaborate with physicians, nurses, social workers, and other healthcare professionals to ensure a coordinated and integrated approach to patient care. Participate in care conferences and case reviews to discuss patient progress and adjust care plans accordingly.
  • Advocacy and Support - Advocate for patients' needs and preferences within the healthcare system. Assist patients in navigating the healthcare system, including scheduling appointments, coordinating referrals, and accessing community resources. Provide emotional support and counseling to patients and families facing challenging health situations.
  • Documentation and Reporting - Maintain accurate and up-to-date electronic health records for each patient. Generate reports on patient outcomes, adherence to care plans, and other relevant metrics. Ensure compliance with regulatory standards and organizational policies.

 

Qualifications

Required Education

  • High School graduate or equivalent.

Required Experience

  • Relevant clinical experience in a healthcare setting.

 

Required Licenses and Certifications

  • Certification as a Medical assistant.
  • BLS Certification.
  • Phlebotomy certification.

Required Knowledge, Skills, and Abilities

  • Strong interpersonal and communication skills.
  • Empathy and a patient-centered approach to care.
  • Ability to work collaboratively in a team environment.
  • Knowledge of relevant medical terminology and healthcare protocols.
  • Familiarity with electronic health record systems.
  • The role may involve working flexible hours, including evenings and weekends.

Physical Demands

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. 

While performing the duties of this job the employee is regularly required to work standing up, walk, use hands to operate tools and equipment and must be able to exert regularly up to 10 pounds of force, frequently exert 30 pounds of force and occasionally exert 50 pounds of force to constantly perform the essential job functions.
