Job Summary

Behavioral health counselors are core members of the Sanitas medical team and provide hope, empowerment, and psychoeducation in order to promote autonomy and self-management by arranging comprehensive mental health care services in a primary care setting.

Essential Job Functions

  • Develop and implement care plans.
  • Complete and maintain all documentation in a timely and accurate manner according to all federal, state and center guidelines.
  • Collaborate with a variety of health professionals in the patient’s circle of care including but not limited to primary care providers, care nurses, care coordinators, community health workers, psychiatrists, and other mental health professionals and specialists.
  • Ensure a comprehensive continuity of care.
  • Provide individual, family or group educational interventions.
  • Guide patients in the development of skills and strategies for dealing with their diagnosis.
  • Assist patients in adjusting to life and making changes.
  • Participate in regularly scheduled weekly caseload consultation and complex case review meetings.
  • Facilitate care plan adjusts for patients who are not improving as expected in consultation with the medical providers.
  • Facilitate referrals to the appropriate provider or in-house intervention program as clinically indicated.
  • Connect patients and family members with community and social resources.
  • Guide clinical staff on how to manage common behavioral health situations.
  • Screen and assess patients for common mental health and substance abuse disorders.
  • Facilitate patient engagement and follow-up care.
  • Support psychotropic medication management as prescribed by medical providers, focusing on treatment adherence monitoring, side effects, and effectiveness of treatment.
  • Promote early detection, accurate diagnosis, personalized treatment, and ongoing support, ultimately leading to improved overall well-being for individuals facing these challenges.
  • Track treatment response and monitor patients for changes in clinical symptoms and treatment side effects or complications.
  • Provide or facilitate in-clinic or outside referrals to evidence-based psychosocial as clinically indicated.
  • Guide clinical staff on how to manage common behavioral health situations.
  • Resolve emergency problems in crisis situations.

Qualifications

Supervisory Responsibilities

This position has no supervisory responsibilities

Required Education

  • Master’s degree in counseling or related field (Social work, Mental health counseling, marriage and couples’ therapy) from accredited program.

Required Experience

  • Relevant experience 1 year.

 

Required Licenses and Certifications

  • BLS certification

Required Knowledge, Skills, and Abilities

  • Able to establish and maintain effective working relationships with mentally or emotionally disabled persons and their families.
  • Flexible schedule weekdays – weekends.
  • Good communication and listening skills.
  • Ability to show empathy and compassion.

Preferred Qualifications

  • Preferably 1 year of experience providing CBT-based interventions with adults with depression and anxiety.

Physical Demands

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.

While performing the duties of this job the employee is regularly required to work standing up, walk, use hands to operate tools and equipment and must be able to exert regularly up to 10 pounds of force, frequently exert 30 pounds of force and occasionally exert 50 pounds of force to constantly perform the essential job functions. The employee will be frequently required to reach with hands and arms, bend, balance, kneel, crouch, crawl, push, and pull. Specific vision abilities required by this job include close vision, distance vision, peripheral vision, depth perception and ability to adjust focus.
