Job Summary

The Patient Care Coordinator is a cross-functional role that supports both the front-end and back-end of the operation creating synergies between Admin & Clinical Ops. This role involves patient interaction, assisting with medical procedures, maintaining patient records, and ensuring the smooth operation of daily clinical functions. Clinical Assistants play a vital role in ensuring a positive experience for patients and supporting the medical team to deliver quality care. Serves members, visitors, and medical staff by identifying the most efficient scheduling methods for members, aiming to optimize the clinic's workflow based on priority needs and predetermined appointment arrangements. As a liaison between visitors, members, and staff, the Patient Care Coordinator facilitates seamless communication and coordination. They utilize various platforms as needed to fulfill their duties effectively.

  • General Duties:
    • Welcome and greet patients, clients, and visitors to the department warmly and courteously. Assess the purpose of their visit and efficiently direct or guide them to the appropriate person or department, minimizing wait times in the lobby. Proactively address and resolve visitor concerns and inquiries to ensure a positive experience.
    • Identify visitors with potentially life-threatening emergencies according to organizational protocols, escort them to the appropriate treatment area, and promptly notify relevant team members.
    • Completes various visitor registration tasks including verifying eligibility, processing forms, collecting visitor payments, and compiling medical information.
    • Safeguard patient visitor confidentiality by securing protected health information (PHI), ensuring PHI is not left in plain sight, and logging off computers before leaving them unattended
    • Provide patients and their visitors with relevant information about their appointments, procedures, and available resources to ensure they have a clear understanding of their healthcare journey.
    • Support administrative tasks such as filing, data entry, and maintaining accurate records to ensure smooth operations within the department.
    • Schedule and confirm patient/members appointments, check-ups, and physician referrals.
    • Provide comprehensive support and assistance to visitors during the check-out process by offering necessary information and resources with a friendly and helpful demeanor, striving to exceed their expectations for a seamless departure experience.
    • Communicates observations of patients' visitors' status to the Manager and/or nurse-in-charge
    • Review medical records and follow up on obtaining missing results prior to patient visitor appointments.
    • Answer telephones promptly and direct calls to the appropriate staff members
    • Maintains a safe, secure, and healthy work environment by adhering to standards, procedures, and legal regulations.
    • Responsible for ordering medical supplies according to departmental need.
    • Perform additional duties as assigned by supervisor or provider.
  • Clinical Duties
    • Assist healthcare providers during examinations, minor surgeries, and other procedures.
    • Take and record vital signs, such as blood pressure, temperature, pulse, and respiratory rate.
    • Perform routine procedures, including wound dressing, specimen collection, and administering injections (if qualified).
    • Prepare examination and treatment rooms by setting up necessary equipment and ensuring cleanliness and organization.
    • Sterilize instruments and equipment, adhering to infection control guidelines.
    • Interview patients, measure vital signs and record information on patient charts.
    • Performs supervised procedures such as but not limited to EKG, vaccines and medications via IM/SC/ID/Oral and assist in procedures/treatments as requested by physician.
    • Checks equipment and supplies prior to use for function, inspection and expiration date.

Qualifications

Required Education

  • High School Graduate or Equivalent

Required Experience

  • Medical Assistant Diploma from a College/University/Technical School or be enrolled in Sanitas MA Apprenticeship Program or a healthcare field diploma (Certified Nurse Assistant, Patient Technician) with 1+ years experience in lieu of MA diploma
  • The position requires candidates to possess at least one year of prior experience in the medical sector, specifically in the capacity of managing a substantial clinic.
  • Any combination of education, training, and experience which demonstrates the ability to perform the duties and responsibilities as described including related work experience.

Required Licenses and Certifications

  • Basic Knowledge of Computer Skills
  • Basic typing proficiency and familiarity with office equipment usage. This entails being able to efficiently operate standard office tools and equipment, such as computers, printers, and scanners, to conduct administrative tasks effectively

Required Knowledge, Skills, and Abilities

  • Bi-lingual in English and Spanish preferred
  • Organizational Skills: Capacity to manage schedules, appointments, and resources efficiently to optimize clinic productivity.
  • Communication: Strong verbal and written communication skills to interact effectively with staff, patients, and other stakeholders.
  • Medical Knowledge: Basic understanding of medical terminology, procedures, and regulations to facilitate effective communication with healthcare professionals and ensure compliance with standards.
  • People Skills: Ability to build rapport with patients and staff, fostering a positive and supportive clinic environment.
  • Diligence: Diligence in maintaining accurate records, adhering to protocols, and ensuring compliance with regulatory requirements.
  • Adaptability: Flexibility to handle unexpected situations and adapt to changing priorities in a dynamic healthcare environment.
  • Customer Service Orientation: Commitment to providing excellent service to patients, addressing their needs promptly and courteously.

Preferred Qualifications

  • At minimum of two years’ experience in customer service in a medical office or large health care setting.

  • Experience: Prior experience in a clinical or healthcare setting is advantageous.
  • Certifications: Basic Life Support (BLS) certification is preferred; additional certifications may be required depending on the healthcare facility and state regulations.
