Qualifications

Position Summary

This position is a Registered Nurse (RN) who performs patient health education at their assigned primary care facility through a unique care program that allow patients to achieve self-management. Together with a Care Promoter Medical Assistant (MA), and under the direction of the Lead Care Program Nurse, the Care Educator (RN), plans, coordinates, and holistically monitor patients’ needs to positively impact their chronic health condition through education. The Care Educator (RN) interacts with patients though a variety of mediums that include onsite face to face, telephonic encounters, and group workshops. The Care Educator (RN) might also possess clerical responsibilities of clinical supplies and vaccine inventory management. This position collaborates with a multi-disciplinary team to assure the highest quality care while focused on the physiological, psychosocial and safety needs of each patient and their families.

  1. Assist with the identification of patients who can qualify for care program
  2. Perform direct and group patient education.
  3. Reviews patient health risk assessments to formulate a nursing care plan.
  4. Complies with documentation requirements of the Care Management program by re-evaluating the care plan with the patient and family/caregiver(s).
  5. Data entry in excel report of patient health information and or supervision of such report.
  6. Preform patient follow up and monitoring according to patient health risk status as stated in program policy.
  7. Promotes patient self-management of disease and behavior modification interventions.
  8. Educates patient and family/caregiver(s) about relevant community resources.
  9. Shares professional knowledge and learning with other team members.
  10. Takes appropriate action to maintain a safe and aseptic environment.
  11. Confers with other members of the healthcare team regarding patient care and work in collaboration towards non-compliant population.
  12. Adheres to department policies and procedures.
  13. Participates in staff and department meetings on monthly basis.
  14. Flexible to changes in work schedule and location as needed by organization.
  15. Communicates to the Care Program Nurse Lead the performance status of their facility’s care program productivity and paraprofessionals working with him/her.
  16. Supervise vaccine and clinical supply orders and inventory as needed per facility.
  17. Maintains effective and efficient use of equipment and supplies.
  18. Attend and assist in company health fairs and events
  19. This position requires some local travel to assist in covering other facilities within the same area.

This position has no supervisory responsibilities.

Physical Demands

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.

While performing the duties of this job the employee is regularly required to work standing up, walk, use hands to operate tools and equipment and must be able to exert regularly up to 10 pounds of force, frequently exert 30 pounds of force and occasionally exert 50 pounds of force to constantly perform the essential job functions. The employee will be frequently required to reach with hands and arms, bend, balance, kneel, crouch, crawl, push, and pull. Specific vision abilities required by this job include close vision, distance vision, peripheral vision, depth perception and ability to adjust focus. The employee is subject to environmental conditions; protection from weather conditions but not necessarily from temperature changes. The worker is subject to noise; there may be sufficient noise to cause the worker to shout in order to be heard above ambient noise level.

Required Education and Experience

  1. BS or Graduate from an accredited school of Nursing
  2. IV Certification required
  3. Active State of Florida Registered Nurse License
  4. Bilingual English/ Spanish
  5. Any combination of education, training, and experience which demonstrates the ability to perform the duties and responsibilities as described including related work experience.
  6. Proficient software computer skills to include WORD, EXCEL and other Microsoft Office programs

Preferred Education and Experience

  1. Case Management experience a plus
  2. CPR-BLS Required; ACLS/PALS preferred
  3. 2 years care management/patient education experience is preferred
  4. Relevant or any other job-related vocational coursework.
