Position Purpose:A Lead RN Educator plays a crucial role in the development and implementation of educational programs within a care-oriented environment. Their responsibilities encompass oversight of the educational curriculum, guiding a team of care educators and health coaches, and ensuring the provision of high-quality care and learning experiences for individuals under their supervision.

Essential Job Functions:

Curriculum Development: Collaborate with healthcare professionals to integrate educational components into clinical interventions. Maintain professional and technical knowledge through workshop attendance; reviewing professional publications. Develop and adapt educational curricula to align with clinical care objectives and individualized treatment plans.

Team Leadership Collaborates with regional team members. Lead the RN Educators/Health Coaches and supervise their engagement activities including, identifying patients, initiating appoints, follow-up appts, communications to the Care Team. Coordinate care and education efforts for the team. Discuss patient progress, educational needs, and adjustments to treatment plans. Make recommendations on additional patient resources or referrals.

Educational Program Implementation Design and deliver educational programs for patients and their caregivers. Provide ongoing support and resources. Enhance understanding of medical conditions, treatment options, and self-care practices. Help patients and caregivers navigate educational aspects related to their care.

Assessment and Progress Tracking Implement assessment tools. Collaborate with healthcare providers. Ensure company population health platforms are utilized appropriately. Evaluate the educational progress of patients within the clinical setting. Address any educational barriers that may impact patient outcomes. Address gaps in knowledge with the Educators/Coaches within the region.

Documentation and Compliance Maintain accurate records of educational interventions, progress, and outcomes. Ensure compliance with regulatory requirements and healthcare standards related to patient education. Compile program result statistics monthly and communicate them to internal/external stakeholders.

Training and Professional Development Conduct training sessions for clinical staff on educational strategies and best practices for patient and caregiver engagement. Stay informed about advancements in healthcare and education. Enhance the professional development of the clinical care team. Provide training and guidance on gaps and new clinical initiatives

Quality Improvement Initiatives Participate in department quality improvement initiatives. Collaborate with the Center on quality improvement initiatives. Enhance the overall effectiveness of educational programs within the Care Programs department. Identify areas for improvement and implement evidence-based solutions.

Performance Monitoring Monitor and ensure that department specific KPIs are being performed and the result communicated to key stakeholders. Track performance on a monthly basis to ensure the region is meeting the established KPIs and Targets.

Interdisciplinary Care Planning Ensure the Care Plan is completed and updated as appropriate. Collaborate with the clinical team on patient care needs. Interreact with Health Plan on complex patients Ensure the regional RN Educators and Health Coaches are following the Care Program playbook and processes. Advise team on resources available through the Health Plan.


Qualifications

Job Experience (years) 2+ years - Proven experience in a leadership role within a primary care office or care setting.

Licenses and Certifications:License Required RN License in Florida or Compact licensure
Certification Required Certification as a care coach preferred

Strong understanding of education principles and evidence-based practices.
Excellent communication and interpersonal skills.
Ability to collaborate with diverse stakeholders, including patients, caregivers, and internal/external partners.
Highly organized
Knowledge of Community resources

Detail oriented

Environmental Conditions:The employee is subject to environmental conditions, protection form whether conditions but not necessarily from temperature changes. The worker is subject to noise; there may be sufficient noise to cause the worker to shout in order to be heard above ambient noise level.

Physical Demands

While performing the duties of this job the employee is regularly required to work standing up, walk, use hands to operate tools and equipment and must be able to exert regularly up to 10 pounds of force, frequently exert 30 pounds of force and occasionally exert 50 pounds of force to constantly perform the essential job functions. The employee will be frequently required to reach with hands and arms, bend, balance, kneel, crouch, crawl, push, and pull. Specific vision abilities required by this job include close vision, distance vision, peripheral vision, depth perception and ability to adjust focus.

Employees are required to wear a mask in common areas. In addition, employees must comply with any state and local masking orders.
