Position Summary
The Care Coach is a national certified medical assistant who works directly with patients in one-to-one encounters and group sessions to reinforce self-management of their chronic condition while close monitoring their established care plan goals.

- Conduct patient face to face visits to promote established care plan goals and reinforce life style modification education
- Works with the Care Educator Nurse to collaboratively discuss cases and ensure patient care gaps are closed
- Identify and see same day patients who will benefit from care education program
- Coach patient group workshops as organized by the Care Program Nurse Manager
- Accurately documents patient information into the EMR system
- Participate in health fairs and health promotion events
- Perform other assigned duties by the Care Program Nurse Manager

Physical Demands
The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
While performing the duties of this job the employee is regularly required to work standing up, walk, use hands to operate tools and equipment and must be able to exert regularly up to 10 pounds of force, frequently exert 30 pounds of force and occasionally exert 50 pounds of force to constantly perform the essential job functions. The employee will be frequently required to reach with hands and arms, bend, balance, kneel, crouch, crawl, push, and pull. Specific vision abilities required by this job include close vision, distance vision, peripheral vision, depth perception and ability to adjust focus. The employee is subject to environmental conditions, protection from weather conditions but not necessarily from temperature changes. The worker is subject to noise; there may be sufficient noise to cause the worker to shout in order to be heard above ambient noise level.
Required Education and Experience
- High school diploma or equivalent and graduation from a nationally accredited Medical Assistant (MA) program with active MA national certification is required.
- Experience in Microsoft Word, Excel, PP.
- This position requires some local travel to assist in covering other facility within the same area.
- Must always have excellent oral and written communication skills with evidence-based data and etiquette
- Ability to think on their feet to solve problems for patients while achieving care program team goals.
- Must be detail-oriented to ensure accuracy of reports and data.
- Ability to maintain effective and organized schedules to ensure timely patient flow.
- Ability to manage stressful and fast passed work environment
- Certified Clinical Health Coach (certification required to be obtained within the first 4 months of employment)
- CPR/BLS for Healthcare Providers is required.
Preferred Education and Experience
- Two (2) years Medical Assistant experience in primary care setting is required.
- Experience working with chronic disease education management is preferred
