Position Purpose

As a Registered Dietitian specializing in the Care at Home population, you will play a critical role in enhancing the health and well-being of bedbound, enteral nutrition-fed, and pressure injury-afflicted individuals with complex chronic conditions. Your expertise will be pivotal in assessing their nutritional needs, creating personalized dietary plans, and conducting physical evaluations to identify signs of malnutrition. This part-time position (18-20 hours per week) requires travel between Miami Dade, Broward, and West Palm Beach to provide essential dietary guidance and support to this vulnerable population.

*Conduct comprehensive assessments of patients' medical history, diagnoses, lifestyle, dietary habits, and existing nutritional support.

*Develop tailored enteral nutrition plans, taking into consideration patients' medical conditions, dietary preferences, and nutritional requirements.

*Collaborate with healthcare teams, including physicians and care coordinators, to integrate nutrition plans into overall patient care plans.

*Perform physical evaluations to identify signs of malnutrition, including muscle wasting, edema, and skin changes like pressure injuries

*Educate patients, caregivers, and family members on proper enteral nutrition administration, dietary modifications, and supplementation as needed.

*Monitor patients' progress and make necessary adjustments to dietary plans based on changes in their health status.

*Maintain accurate and thorough documentation of patient assessments, interventions, and outcomes.

Qualifications

Job Experience/ Education (years)
* BA/BS Degree

* Bachelor’s degree from an accredited program through ACEND, Master's degree preferred

* Completion of an accredited dietetic internship program

* Minimum 5 years working with acutely ill patients, preferably in a hospital setting.

* Experience in providing nutritional care to individuals with complex chronic conditions and enteral nutrition needs.

* Licensed in the state of Florida

Other Knowledge, Skills and Abilities:

* Proficiency in conducting nutritional assessments and creating individualized dietary plans.

*In-depth knowledge of enteral nutrition, supplementation, and therapeutic diets.

*Familiarity with pressure injury prevention and wound healing nutritional considerations.
*Strong interpersonal skills for effective patient interaction and collaboration with healthcare teams.

*Excellent communication skills, both verbal and written

*Ability to adapt assessment techniques for patients with limited mobility.

*Empathy, sensitivity, and cultural competence in dealing with diverse patient populations.

*Strong organizational skills to manage a varied caseload and maintain accurate documentation.
