Lakeview Village in Lenexa, Kansas is a not-for-profit continuing care retirement community that offers sub-acute rehabilitation and long-term care, celebrating its 60th anniversary and 5th straight year as the US News and World Report's Best Short Stay Rehab.
Job Details:
- Works as the case manager/social worker in a 38-bed short-stay rehab.
- Works with insurance companies on authorization and communicates updates.
- Advocates for clients' needs with discharge plans to ensure safe discharges to the community.
- Promotes the psychosocial well-being of the residents by supporting resident-centered care.
- Assists in interpreting the social, psychological, and emotional needs of the residents and their families to the interdisciplinary team involved with the residents’ plans of care.
- Works closely with the interdisciplinary team to ensure that the residents live in a safe, comfortable, and functional environment.
- Ensures that all appropriate related documentation is maintained and compliant with Local, State, and Federal regulations.
