Lakeview Village is searching for a caring and dedicated full time Licensed Social Worker to join our excellent team of employees for our short-stay rehab.


Lakeview Village in Lenexa, Kansas is a not-for-profit continuing care retirement community that offers sub-acute rehabilitation and long-term care, celebrating its 60th anniversary and 5th straight year as the US News and World Report's Best Short Stay Rehab.


Job Details:

  • Works as the case manager/social worker in a 38-bed short-stay rehab.
  • Works with insurance companies on authorization and communicates updates.
  • Advocates for clients' needs with discharge plans to ensure safe discharges to the community.
  • Promotes the psychosocial well-being of the residents by supporting resident-centered care.
  • Assists in interpreting the social, psychological, and emotional needs of the residents and their families to the interdisciplinary team involved with the residents’ plans of care.
  • Works closely with the interdisciplinary team to ensure that the residents live in a safe, comfortable, and functional environment.
  • Ensures that all appropriate related documentation is maintained and compliant with Local, State, and Federal regulations.

Qualifications

  • Must be licensed as a Social Worker in the State of Kansas, Master's Degree is preferred.
  • Current CPR certification is required.
  • Must demonstrate strong interpersonal skills while working in a team-oriented setting.
  • Must demonstrate an effective understanding of Microsoft Office Suite software.
  • Demonstrates good communication skills, including but not limited to effective reading, writing, speaking, and analytical skills, with a fluent understanding of English.
  • Possesses an empathetic approach to the elderly.
  • Able to work in a team setting with residents, relatives, staff, and others.
