Position Summary: The primary function of this position is to serve as a resident advocate so as to ensure each residents psychosocial, spiritual needs and wishes are met and maintained from the time of admission through discharge. The functions are provided in accordance with all federal, state and local standards, guidelines and regulations which govern the facility, and, as directed by the Social Services Director so as to ensure accurate rights and needs of each resident is met on a continuum.
Duties and Responsibilities – Social Worker
General: 75%
1. Work with skilled nursing staff to ensure a timely and smooth admission/discharge process.
2. Is assigned to floors and residents determined by Social Service Director.
3. Competes all discharge planning making appropriate referrals upon discharge for the residents continuing care.
4. Works with nursing staff regarding resident room changes as necessary or requested.
5. Counsels residents and families need.
6. Assists residents and significant others with the adjustment process to the facility.
7. Promotes public relations in the community.
8. Participates on the Interdisciplinary Care Committee and completes the initial assessment MDS and care plan for each resident.
9. Evaluates all psychosocial needs of the resident and creates a plan of care addressing any such.
10. Records in progress notes, any changes in condition and/or psychosocial needs.
11. Maintains a safe work environment and exhibits safe work practices, assuring resident safety.
Administration: 25%
12. Follows department budget.
13. Completes paperwork for all new admissions and discharges in a timely manner.
14. Completes necessary paperwork and maintenance of records.
15. Provides input for reviews, revisions and establishment of new policies and procedures for the department(s).
16. Adheres to, upholds and enforces all facility policies and procedures.
17. Adheres to, upholds and enforces all regulations and recommendations of the Department of Health.
18. Keeps apprised of new information in the field by attending seminars/affiliation meetings, reading related literature, etc., as directed by the Social Services Director.
19. Serves on committees as assigned.

22. Actively participates in all aspects of overall facility management and thus is cooperative with co-workers and peers.

Working Conditions:
1. Works indoors in limited office space, adequate lighting, and temperatures, but subject to fluctuations in ventilation.
2. Works with average indoor exposure to noise, stress and disruptions.
3. Normal indoor exposure to dust/dirt.
4. Periodically works beyond normal work hours or on an as-needed basis.
5. Potential exposure to blood borne pathogens or communicable diseases.
6. Works in conditions of potential outbursts of disruptive behavior of residents.
