LifeSTEPS is one of the largest providers of social services for affordable and permanent supportive housing communities in California, empowering individuals and families through supportive housing and health programs. We are seeking a dynamic and experienced leader to join our team as the Cal-AIM Services Director.  This full-time, exempt position will be based at our LifeSTEPS Corporate Office in Sacramento, California, providing strategic leadership and direction for our impactful Cal-AIM initiatives.

 

Position Overview
 

The Cal-AIM Services Director leads the delivery and management of Enhanced Care Management (ECM), Community Supports (CS), and Community Health Worker (CHW) services for Medi-Cal eligible adults (18 and older).  Initially, referrals will come from Managed Care Plans (MCPs) contracted with LifeSTEPS, with future expansion to include referrals from LifeSTEPS’ own affordable and permanent supportive housing residents who are Medi-Cal and Medicare eligible.  This role ensures comprehensive and quality service provision, bridging healthcare and housing supportive systems for vulnerable populations. 

 

Key Responsibilities include, but not limited to:

  • Assist the Deputy Director and reports directly to the Supportive Health and Housing Director regarding all program aspects, including development, delivery and performance of Cal-AIM Services (Enhanced Care Management, Community Supports, and Community Health Workers) in Los Angeles and Sacramento Counties.
  • Assist the Deputy Director with the reporting, forecasting, and all other administrative aspects of the CITED-PATH grant.
  • Support the expansion of Cal-AIM services to additional California counties during the 2024-2025 period.
  • Ensures administrative compliance with all program activities, while developing and implementing plans to continually meet productivity within budget.
  • Directs the delivery of Cal-AIM services which includes the supervision of the outreach and engagement, intake process and completion of case records to ensure documentation meets agency and funding sources standards.
  • Responsible for hiring, development and performance of support and professional staff ensuring the CITED-PATH grant and services contracts with all Managed Care Plans (MCPs).
  • Ensures compliance and contractual obligations are met and minimizes disallowance of funds; provides on-site training to support staff in meeting workload requirements.
  • Leads and participates in the development and submissions of Request for Information (RFIs), Letter of Interest (LOIs), Requests for Quotes (RFQs), Requests for Proposal (RFPs) and/or Grant Requests.
  • Develops and maintains a collaborative relationship with agency network, county and state officials, funding sources and CPI (Collaborative Path Initiative) network.

Qualifications

Education and/or Experience
The minimum qualifications and training for this position are:

  • A Master’s degree and 5+ years of management experience in social, health, or housing services, OR a Bachelor’s degree with 10+ years of management experience in these fields.
  • At least 5 years of experience managing and delivering Cal-AIM programs, including ECM, CS, and CHW services. This includes overseeing functions such as outreach, navigation, enrollment, health and wellness sessions, referrals to medical services, housing deposits, location, and retention.
  • At least 5 years of experience working with underserved populations, homeless, and low-income individuals and families.
