About the Position
The Cal-AIM Project Supervisor will provide regional administrative and supervisory support to the Cal-AIM Services Director and manage the Cal-AIM programs within the region. The Cal-AIM Project Supervisor is expected to exercise discretion and independent judgment when implementing and coordinating tasks for the project and must have the ability to handle shifting priorities and multiple projects independently.
The Cal-AIM Project Supervisor represents and is assigned as the ECM and/or Community Support Director for Medical Care Providers (MCP’s).
Core Supervisory Duties:
- Cultivate, articulate and live out the LifeSTEPS workplace values of integrity, innovation, empowerment, respect and personal growth in tandem with the Oz Principle.
- Directly supervise two or more non-supervisory employees within the assigned region.
- Fulfill supervisory responsibilities in accordance with the organization's policies and applicable laws, specified grant requirements, and specified contract/program requirements.
- Interviewing, hiring, and training new employees.
- Planning, assigning, and directing workload.
- Evaluate staff performance.
- Address staff complaints and resolve problems.
- Oversee onsite programs and conducts quarterly onsite visits to properties to ensure agency meets standards and procedures criteria.
- Review database entries, monthly service reports, and any required reports; ensure they are accurate and submitted by applicable due dates.
- Review case records, timecards, and expense reimbursement report.
- Develop resources, including donations, for the resident community.
- Work with the LifeSTEPS Medical Biller to ensure all required documentation is submitted.
- Implement programs for Members based on the community needs identified.
- Implement specific programs as detailed within contractual agreements.
- Maintain all Member services in the mylifesteps.org database.
- Maintain and report statistical tracking of Members receiving services.
Core Lead Care Manager Duties:
- Ensure Cal-AIM Lead Care Management services follow all procedures and policies.
- Work closely with LCM’s and other supportive service agencies, including case management, care management, physical and mental health providers to ensure Members’ needs are being met.
- Evaluate crisis situations as they arise.
- Coordinate with community organizations, public agencies, and volunteers to ensure Member access to resources.
- Provide training for Lead Care Managers, including initial training and additional scheduled trainings as needed and/or required by the Medical Care Provider.
- Visit sites or locations where Members receive service to observe effectiveness of care management; provide feedback; complete quarterly compliance audits.
- Shadow Lead Care Manager to observe effectiveness of care management; provide feedback; complete quarterly compliance audits.
- Cover the duties of Lead Care Managers to ensure continuity and delivery of services to members.
