POSITION OVERVIEW:
We are seeking a Peer Case Manager to join the team of our TAY Restorative Reentry Program. The Peer Case Manager provides comprehensive case management to adults 18 to 25 years of age who are impacted by the Criminal justice system and who may be diagnosed with mental illness and/or substance use disorder. Comprehensive case management and peer navigation services will take place via interviews with clients and family members; initial and ongoing psychosocial assessments and linking clients and helping them maintain connection to community providers and resources. The Peer Case Manager also works collaboratively with the Program Manager to address client needs and receive guidance and support. The Peer Navigator will have knowledge and understanding of restorative justice and Cognitive behavioral therapy.
Due to their lived experiences within the criminal justice system, Peer Navigators have a unique ability to relate to and motivate clients through the shared reentry experience, providing outreach engagement, linkage, and ensuring they receive necessary services, including restorative justice and cognitive behavioral therapy. This role will uphold and champion the agency’s mission, vision, and values and contribute to a collaborative and inclusive work culture.
Essential Job Functions and Responsibilities:
- Perform regular assessments of clients' needs in the following areas: housing, employment, substance use, mental health, coaching, coordination of care, social and family relationships.
- Make regular visits to clients to check and track for progress. As well as monitor for at risk behaviors and conduct safety plan risk assessments as needed, notifying supervisory staff of findings.
- Knowledge of Alameda County social service resources, housing resources, community resources and build relationships with resource providers.
- Provide outreach engagement, linkage, and follow up services to clients.
- Serve as a role model to inspire, engage, and advocate for clients rights.
- Participate in weekly multidisciplinary team meetings, monthly training sessions, and other meetings as required
- Collaborate with the Program Supervisor and Clinician during daily reviews on a client to client basis to debrief and establish service recommendations
- Provide complete and thorough documentation, e.g. Salesforce Peer Assessment, progress notes, and report client progress to supervisors and enter into Salesforce database
- Respond to work-related calls and emails in a timely manner
- Utilize motivational interviewing and other interventions to assist clients
- Maintains all records in accordance with the Quality Assurance standards of Alameda County Probation Department.
- May serve as a liaison to other community agencies.
- Familiar with restorative justice practices
- Has a firm understanding of the principles, components, and benefits of Cognitive Behavioral Therapy.

- Attend trainings and participate in events relevant to key responsibilities

- Leads and collaborate on special projects related to agency objectives as appropriate
