POSITION OVERVIEW:

The Clinician will share a caseload of IHOT clients and meet monthly productivity goals; attend weekly group and individual clinical supervision; participate in weekly trainings as assigned by his/her clinical supervisor and LFCS administration; and attend weekly Outpatient Mental Health Staff meetings. The Clinician will work within all compliance standards; complete documentation in a timely manner per LFCS policy; and maintain strict confidentiality protocols. The Clinician is also tasked with the responsibility of case management work. It is possible to accrue supervised clinical hours required for license as a MFT, LCSW, or Psychologist. Essential Job Functions and Responsibilities:

  • Conducts community outreach to homeless clientele and clientele residing at various institutional settings throughout the East Bay area who are enrolled in the IHOT program in an effort to establish contact, build rapport, and provide linkage services.
  • Provide services in the field and at various institutional settings and locations in the community, as necessary, as well as transport clientele to necessary appointments and community resources and supports.
  • Meets with clients on a consistent basis or as needed in the field for the purposes of establishing and maintaining contact; gradually build rapport and therapeutic alliance with clients; facilitate client interests in resources; develop and implement client care plans; support and accompany clients through interpersonal and institutional transitions; teach and enhance coping skills; facilitate healthy dialogue between clients and respective family members or relevant parties; leverage the interests of family members and clients towards common goals; and provide referral and linkage services to specialty mental health services in the community.
  • Provide case management duties for clients and connect them to various resources as necessary, including, but not limited to, assisting clients to apply for General Assistance and Social Security Income; initiate and follow-up with referrals to temporary shelters and long-term housing; referral and linkage to services available within Alameda County Behavioral Health (ACBH) as well as other necessary community resources and supports.
  • Consistently collaborate with mental health and other social service professionals in various institutional capacities and settings to initiate, refer, and follow-up with services in conjunction with or on behalf of clients.
  • Provides and implements brief evidenced-based therapeutic interventions with clients and their respective families in regards to various psychotherapeutic modalities, including, but not limited to, clinical case management, structural family therapy, cognitive-behavioral therapy, motivational interviewing, mindfulness, trauma-informed care, and dialectical behavioral therapy.
  • Conducts clinical consultation to IHOT staff in regards to the symptomologies of various psychological disorders and provides input and feedback regarding appropriate therapeutic approaches and interventions.
  • Conducts clinical interviews with clients and their respective families as a means to evaluate and implement appropriate short-term therapeutic interventions which will strengthen client and family buy-in to services, build rapport and therapeutic alliance, and enhance efforts of referral and linkage to services.
  • Provides psycho-education to relevant stakeholders involved the lives of clientele, including, but not limited to, family members, relevant community members, and other professionals
  • Consistently write case notes regarding services rendered in a timely manner and in compliance with standards of care of La Familia IHOT.
  • Consistently attend weekly meetings of IHOT Staff Meetings/Group Supervision; weekly meetings of individual supervision with the Program Supervisor; monthly ACBH meetings; and other meetings or trainings relevant for community or professional stakeholder purposes or professional development.
  • Serve as a liaison with staff of ACBH in coordinating services with ACBH and our IHOT program.
  • Assist Program Supervisor in periodically reviewing ACBH webpage for up to date information regarding Alameda County policies, procedures, trainings, and announcements.
  • Maintain client confidentiality and charts in compliance with Health Insurance Portability and Accountability Act (HIPAA)
  • Serve as On-Duty Clinician during designated times of business hours of La Familia Clinic.
  • May serve as a liaison to other community agencies

  • Attend trainings and participate in events relevant to key responsibilities
  • Track and regularly report on key metrics for responsible function areas
  • Leads and collaborate on special projects related to agency objectives as appropriate

Qualifications

QUALIFICATIONS & EXPERIENCE:

  • Graduate degree in social work, psychology or related field, and registration with the California BBS/BOP (e.g. PhD, PSYD, LCSW, MFT, ASW, and AMFT) or being currently enrolled in a graduate program for mental health and having completed half of the program.
  • Experience working with serious mental illness and culturally-diverse populations is strongly desired.
  • Knowledge of principles and practices of case management, including outreach and engagement, motivational interviewing, harm reduction, resource coordination, linkage and referral, individual rehabilitation, and medication support.
  • Non-Profit and Community Based Organization experience highly desirable.
  • Demonstrates and models excellent “customer service” orientation, especially in the ability to interact with people at all levels within and outside the agency.
  • Thinks independently and can make informed decisions when needed.
  • Exercises extreme discretion with the ability to hold, balance and transmit public and confidential information needed by various parties.
  • Proficient using MS Office products, particularly Word, Excel, Outlook and PowerPoint.
  • Acquires or maintains proficiency in using Google Drive products, including Docs, Sheets, and Forms.
  • Valid CA Driver’s license
    • Auto insurance:  Personal vehicle may be used for business related purposes; it is recommended you inform your auto insurance carrier.
