POSITION OVERVIEW:
The Licensed Vocational Nurse is primarily responsible for performing outpatient primary care, monitoring of key health indicators, and taking vital signs of patients who meet criteria for services under the Certified Community Behavioral Health Center (CCBHC). Nine Core Services are offered either directly or through formal partnerships: Crisis Services, Screening, Assessment, and Diagnosis, Person-Centered and Family-Centered Treatment Planning, Outpatient Mental Health and Substance Use Services, Primary Care Screening and Monitoring, Targeted Case Management Services, Psychiatric Rehabilitation Services, Peer Supports and Family/Caregiver Supports, Community Care for Uniformed Service Members and Veterans. The Licensed Vocational Nurse will work closely with all partners including internal La Familia teams, key stakeholders, community organizations, county social workers, parents and team members, consulting and collaborating on client’s progress and treatment plans. Essential Job Functions and Responsibilities:

- Conduct assessments, planning, implementation and evaluation of patient care
- Make determinations whether a patient needs a physician (prepare for the call using the SBAR (Situation, Background, Assessment, Recommendations) or a determination, with the hospital and/or emergency room RN, that the patient should be sent to the emergency room.
- Recognize, report, and record any change in condition or behavior of the patient.
- Accurately communicate in writing through utilization of appropriate charting method (Electronic Health Record), the patient care summary and proper transcription of medical orders.
- Establish therapeutic rapport with patients, families, and walk-in community members, while respecting patient's rights to privacy and confidentiality of information.
- Communicate pertinent information regarding patients with the team
- Support patients in registering on the testing lab website
- Refer out for COVID-19 testing
- Place follow up phone calls to communicate test results and/or discrepancies, as needed
- Provide patient information as necessary
- Complete and document medication count as required and necessary.
- Monitor patient’s post-administration for adverse effects and provide treatment and facilitate emergency response as needed
- Prepare and perform blood pressure screenings
- Prepare and perform A1C and blood glucose screenings
- Weight Assessment and Counseling for Nutrition and Physical Activity for children/Adolescents
- Take, monitor, and control High Blood Pressure for clients
- Provide patient education and supporting materials.
- Will also provide training and psychiatric education to adults and families, but also stakeholders including internal staff, community partners, and similar as directed; as well as conduct outreach activities to increase knowledge of and engagement in services either for existing clients or as a standalone treatment
- Will participate in program planning and utilization review and quality assurance, and will follow Drug Medi-Cal Organized Delivery System charting and documentation requirements and Agency regulations.
- Demonstrate understanding of emergency situations and response plans.
- Answer patient questions and refer to Providers as necessary
- Following all site equipment storage and security protocols
- Communicate with site leaders regarding supply needs
- Support facility management and daily maintenance routines, including cleanliness and sanitation protocols
- Prioritize patient and staff safety at all times, following protocols and procedures as communicated by site leadership.

- Attend trainings and participate in events relevant to key responsibilities
- Track and regularly report on key metrics for responsible function areas
- Leads and collaborate on special projects related to agency objectives as appropriate
