SUMMARY OF POSITION:

 

Working under the supervision of the PACE Center Manager, the PACE OT is independent in the application of occupational therapy knowledge and skills. Primary duties include assessment and care planning, care coordination with the PACE Interdisciplinary Team (IDT), home safety assessments, and oversight of a comprehensive therapy program for NEMS PACE participants in partnership with the NEMS PACE Physical Therapist.

 

ESSENTIAL JOB FUNCTIONS:

  • Conducts initial and periodic occupational therapy assessments to determine PACE Participants’ care needs, preferences, and goals. Establishes realistic and measurable goals and sets up treatment plans to administer rehabilitation treatment. 
  • Participates in the development and revision of the participant’s plan of care as an active member of the interdisciplinary team.
  • Remains alert to pertinent input from other team members, Participants, and caregivers and updates the IDT promptly regarding any changes in Participants’ condition or medical status.
  • Oversees skilled and therapeutic exercises in the PACE Rehabilitation Gym ensuring that therapy conforms to accepted industry practice standards and organizational/department standards.
  • Provides Participant and family education in rehabilitation needs and interventions.
  • Works with other therapists and primary care staff to provide comprehensive, integrated Participant care.
  • Documents all occupational therapy treatment and interventions accurately and within departmental time requirements into Participants’ electronic health record.
  • Understands and demonstrates respect for Participants’ rights and utilizes established mechanisms for management of ethical issues in Participant care.
  • Plans and implements home-based interventions, including installation of adaptable equipment for Participant safety. Conducts required home safety visits, and other visits, as necessary for home-based interventions and observation.
  • Provides supervision to program aides and other staff who are implementing occupational therapy treatment and/or maintenance programs.
  • Demonstrates active participation in QI processes, staff training, and meetings.
  • Provides care to Participants in-person, via telehealth, and/or via telephone, as indicated.
  • Provides home health as needed.
  • Supports enrollment of prospective Participants in the PACE program.
  • Utilizes Epic appropriately and efficiently and serve as content and workflow expert for resolving issues and proposing solutions in Epic.
  • May provide after-hours on-call coverage to support PACE participants when the PACE Center is closed as needed for support.
  • Performs other job duties as required by manager/supervisor.

 

QUALIFICATIONS:

 

Required:
 

  • Graduate of an Occupational Therapy program accredited by the American Occupational Therapy Association
  • Valid and current California Occupational Therapy license in good standing
  • Current BLS and First Aid required
  • Knowledge of proper body mechanics to avoid work-related injuries, and the ability to educate other staff on proper body mechanics.
     

Preferred:
 

  • At least one year of clinical experience working with the older adult population.
  • Advanced Practice in Dysphagia strongly preferred.
  • Knowledge of therapy equipment and technology, including aerobic and strength conditioning equipment tailored to the older adult population and use of mechanical assist devices.
  • Minimum two years clinical experience with adults with as an Occupational Therapist.
  • Valid CA driver’s license with good driving history (last 5 years)

 

LANGUAGE:
 

  • Must be able to fluently speak, read and write English.
  • Fluency in Chinese (Cantonese) preferred.
  • Fluency in other languages is an asset.

 

STATUS:

  • This is an FLSA Exempt position.
  • This is not an OSHA high-risk position.
  • This is a Full Time position.
