POSITION SUMMARY:
Person in this position will play a critical role in delivering comprehensive healthcare services to
individuals seeking assistance with substance abuse disorders. Working within a multidisciplinary team, the APRN assesses, diagnoses, and treats patients, employing a holistic approach that integrates medical, psychological, and social aspects of care. The APRN is responsible for providing medication-assisted treatment (MAT) and coordinating various therapeutic interventions to support patients on their journey to recovery. Essential Job Functions include those listed below.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
  • Conduct thorough assessments of patients to determine eligibility for medially assisted treatment.
  • Gather and analyze medical histories, substance use histories, and relevant psychosocial information.
  • Collaborate with other healthcare professionals to ensure a comprehensive understanding of each patient’s needs.
  • Prescribe and manage medications for substance use disorder treatment, such as opioid agonist therapy (e.g., vivitrol, buprenorphine).
  • Monitor medication adherence, efficacy, and potential side effects.
  • Adjust medication regimens based on patient response and clinical judgment.
  • Collaborate with treatment team to develop and implement individualized treatment plans.
  • Participate in case conferences and treatment team meeting to ensure coordinated and holistic care.
  • Communicate effectively with patients and their families to foster engagement and adherence to treatment plans.
  • Provide education to patients on the nature of substance use disorders, treatment options, and recovery strategies.
  • Conduct counseling sessions, addressing both medical and psychosocial aspects of addiction.
  • Foster a therapeutic alliance with patients to support their commitment to recovery.
  • Maintain accurate and up-to-date patient records, documenting assessments, treatment plans, and interventions.
  • Ensure compliance with legal and regulatory requirements related to medical record documentation.
  • Participate in quality improvement initiatives to enhance the effectiveness of the Medically Assisted Program.
  • Stay abreast of evidence-based practices and contribute to the integration of best practices into the program.
  • Must Embrace and embody the mission, vision, guiding principles, clinical vision, and goals of WestCare Foundation; and
  • Perform all other duties as assigned.

Qualifications

ESSENTIAL QUALIFICATIONS:

To perform this job successfully, an individual must be able to perform the duties and responsibilities satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required.

Experience and Competencies:

  • Experience in addiction medicine or substance use disorder treatment is strongly preferred.
  • Strong interpersonal and communication skills.
  • Ability to work collaboratively within a multidisciplinary team.
  • Knowledge of and commitment to evidence-based practices in addiction treatment.
  • Bilingual (English/Spanish) is preferred.
  • Adherence to the highest standard of ethical conduct, especially to standards governing confidentiality.
  • Professional appearance and demeanor.
  • Must be sensitive to populations served.
  • Must be able to obtain and maintain a level II clearance.
  • Valid driver’s license and ability to obtain and maintain WestCare’s vehicle insurance is required.
  • During your tenure with WestCare, there may be new requirements, including, but not limited to vaccinations that are issued by local, State, Federal, and/or Funders that WestCare may have to comply with. Should this occur Human Resources or appropriate personnel will inform you.
  • CPR Certification and First Aid Certification required.

EDUCATION and/or EXPERIENCE:

  • Master's degree in Nursing with a focus on psychiatric-mental health or adult health.

LICENSURE/CERTIFICATION:

  • Current licensure as an Advanced Practice Registered Nurse (APRN) in the relevant jurisdiction.
  • Certification in psychiatric-mental health or addiction nursing (PMHNP or ANP-BC) is preferred.

WORKING CONDITIONS:

  • Work is primarily performed in an office or in the MAT Mobile Van.
  • This position may involve flexible hours to accommodate patient needs and may require occasional travel for trainings or meetings.
