Position Summary:
The Family Support Worker is responsible for transporting clients and supervising visits between clients and their family members to ensure a safe and supportive environment. This role involves observing visits impartially, documenting interactions, and entering necessary information into required systems in a timely manner. The worker also assists families with visitation plans and schedules, provides orientation at the visitation center, and communicates effectively with case management staff. Additionally, the Family Support Worker reports any incidents or accidents in accordance with organizational policies.
Essential Job Functions:
- Transports clients.
- Impartially observe visits between clients and family members and documents observations and enter documentation into the required system.
- Completes administrative tasks related to the functions and responsibilities of the Family Support Worker in a timely manner including entry of medical/dental records within 72 hours of visit, and verbal/written communication with case management staff.
- On call responsibilities include transporting, supervising clients, and assisting Full Case Managers.
- Provide dependable transportation, visitation and logistical support to operational teams.
- Facilitate, intervene and terminate a visit, as necessary.
- Oversee that visit rooms are neatly maintained and safe for visits.
- Facilitate and assist with orientation of families in visitation center; assist with procedures for visits prior to initial visitation.
- Set up a visitation plan with each family utilizing the center.
- Set visitation schedules for families.
- Directly transport clients to visitations, medical appointments, counseling sessions, shopping and other necessary activities.
- Assist in program data collection and data entry to maintain the appropriate records.
- Report incidents and accidents within required timeframes to keep with organization policy and procedures.
- Other duties as assigned.
