Licensed Practical Nurse (LPN)

Position Summary:

Person in this position will provide medical services to clients.  Essential Job Functions include those listed below. Other duties may be assigned as needed.

Essential Job Functions:

• Provides medical screenings and examinations on WestCare clients.  Makes determination of appropriate acute care setting based on symptoms, BAC, and behavioral indicators.
• Cares for patients with common illness or minor injuries and/or refers patients to appropriate medical setting for health treatment.
• Utilize and manage Electronic Health Record (EHR) systems for data entry and client information tracking, ensuring program alignment.
• Observes, determines, reports and records changes in objective signs and subjective symptoms.
• Participates in multidisciplinary treatment team, participates in assessments of clients admitted to  program, to include physical, social, medical, substance abuse and mental health assessments, and in the development of treatment and placement plans.
• Oversight of self-administration of patient medications at the facility, provide non-prescription medications as needed, and evaluation of on-going health status of clients.
• Medication Administration training for all staff.
• Client education on medication and health related issues.
• Inventory medications monthly and properly dispose of medications (following medication disposal protocol) as necessary.

• Insure completion of and adherence to all safety guidelines.
• Maintain medical portion of clinical record.
• Work with area medical personnel to facilitate compliance with all recommendations.

• May require local travel.

• Other duties may be assigned as needed.

Essential Qualifications:

• Current Licensed Practical Nurse Status
• 2+ years’ experience working with adults in a detoxification, homeless or residential setting.
• Must pass a DCF Level 2 background clearance.
• During your tenure with WestCare there may be new requirements, including, but not limited to vaccinations that are issued by local, State, Federal, and/or Funders that WestCare may have to comply with. Should this occur Human Resources or appropriate personnel will inform you.

Education:

• Completed Practical Nursing Program

• Indoor and outdoor setting
• Fast paced environment.
• Potential for high noise level at times

• All procedures or other job-related tasks that involve an inherent potential for mucous membrane or skin contact with human blood, body fluids, or tissues, OR a potential for spills or splashes of them are Category I tasks. Use of appropriate personal protective equipment will be required for every employee engaged in Category I tasks.
