Position Summary:
In general, the Prevention Specialist functions as the primary client advocate, keeps abreast of progress, and coordinates all services delivered to the participants both internally and through external referral.
Essential Job Functions:
- Responsible for insuring program participants patients receive all appropriate services;
- Coordinate services provided by ancillary agencies; coordinates and participates in treatment planning and reviews with the entire treatment team;
- Consults with each patient concerning attainment of treatment plan goals and objectives;
- Conducting community-based outreach activities;
- Provide HIV and Hepatitis testing and counseling testing and linkage;
- Develop Individualized risk reduction Prevention Plan in collaboration with the consumer; documenting all sessions in the clinical record;
- Provide HIV risk reduction counseling (using evidence-based practice);
- Implement evidenced based intervention;
- Gain certification to provide youth and young adults the evidence-based community
- Participate in the development and implementation of the environmental strategies;
- Document all service delivery in clinical record in accord with Federal, State, and agency standards;
- Participate in all staff meetings and training as assigned by the Program Coordinator;
- Participate in Performance Improvement activities;
- Transport clients when necessary;
- Work in collaboration with Program Coordinator to co-facilitate support groups and other activities;
- Embrace and embody the mission, vision, guiding principles, clinical vision and goals of WestCare Foundation; and
- All other duties as assigned.
