Description

Provide Case management service to clients as outlined in the case management procedure manual in accordance with Children and Families Rules and Regulations.

  • 1.0 Provide a minimum of 115 hours of service for assigned clients each month.
  • 2.0 Develop and maintain a written assessment and service plan for each client.
    • 2.1 Written assessment and service plans are developed with as much participation from the client as possible and reasonable.
    • 2.2 Service plans and assessments are maintained in accordance with the Center's protocol and procedures for frequency and timeliness for completion of this document.
    • 2.3 Service plans and assessments are appropriately signed and properly credentialed.
  • 3.0 Conduct case management activities for assigned clients to include assessment of strengths and needs, service goals, service planning, client advocacy, and case monitoring.
    • 3.1 Case Management activities are performed in accordance with Center policy and procedures.
    • 3.2 Case Management activities are reflective of the services specified as being of need in the written service and assessment plan.
    • 3.3 The relationship between the needs identified in the assessment and the goals identified on the service plan will be clearly documented in the progress note that describes each unit of service provided to a consumer.
  • 4.0 Prepare written documentation of each service conducted.
    • 4.1 Written documentation for each service conducted is performed in accordance with the Center's standards for service documentation in the client's medical record.
    • 4.2 Correctly completed service activity logs with progress notes attached are submitted to the finance department on a daily basis.
    • 4.3 Progress notes accurately reflect the nature of service provided, activities provided, and the timeliness of services and outcome.
  • 5.0 Conduct all activities in such a way as to safeguard the confidential status of client information.
    • 5.1 All "on the job" and "off the job" activities are performed in accordance with the Center's confidentiality standards.
  • 6.0 Participates in administrative, clinical staffing, staff development and other meetings as scheduled.
    • 6.1 Meetings are regularly attended.
    • 6.2 Maintains up-to-date knowledge of the Children's and Families Procedural Standards.
    • 6.3 Completes all orientation and mandated training activities according to criteria established for the position.
    • 6.4 Participates in relevant staff development activities.
    • 6.5 Must obtain and maintain Children and Families certification as a Case Manager.

Qualifications

Experience and Competencies:

  • Two years of related work experience in Mental Health, and / or Substance Abuse field preferred
  • Ability to meet requirements of Monroe County Sheriffs Office background check
  • CPR Certification, First Aid Certification and an annual Tuberculosis test.
  • Ability to provide an Affidavit of Good Moral Character
  • Clearance in Pre-Employment Drug Testing and Criminal Records Checks, as required.
