What are we looking for in a Care Coordinator?
Care Coordinators are part of the WISe (Wraparound with Intensive Services) program, which serves youth who have mental health diagnoses and are experiencing severe behavioral disruptions that impact their functioning in family, school, and social environments. WISe is an evidence-based model with a collaborative, cross-system approach to creative problem-solving. Care Coordinators work with youth and their families to develop and implement individualized, cross-system care plans that work toward the family’s long-term goals. Services are provided where and when convenient for the family, sometimes out in the community and outside of regular business hours. Care Coordinators must be skilled in managing team conflict, engaging a variety of personalities, facilitating meetings, and communicating with various audiences. This position will have a varying work schedule of mostly Monday-Friday day shift, but also requires work outside of standard business hours to meet the needs of client families served. Although based in our Yakima outpatient center, Care Coordinators may meet with individuals and families out in the community at a location that's most convenient for the family, so regular travel throughout Yakima County is expected. Â
Care Coordinator duties may include:
- facilitating meetings between the client, family, and treatment team
- acting as a partner to the family and as liaison with community providers, school representatives, legal system, and support systems
- collaborating with the family & treatment team to develop a cross-system care plan and updating the plan after each child & family meeting
- completing CANS & SNCD assessments
- providing on-call crisis coverage and routine services outside of regular business hours
- following WISe protocol with fidelity to the model
- assessing clients for safety and risk to self or others and taking appropriate crisis intervention steps when indicated
- completing all documentation using strengths-based language
- providing WISe services and crisis coverage outside of regular business hours
Qualifications:
- Bachelor's degree in social work, psychology, or related field
- Must be eligible for registration as Agency Affiliated Counselor
- Must have valid driver's license, vehicle liability insurance, and a minimum of two years of safe driving experience
- In compliance with state and federal law, Comprehensive Healthcare requires all candidates to complete a post-offer, pre-employment drug screen and background check.
