Director of Supported Community Living

 

Location:  Corporate Office – Kirkwood Avenue                 Reports to:  Chief Operating Officer

 

Division:  Administration                                                              Type:  Full-time, Salaried, Exempt, Supervisory

 

Primary Duties and Responsibilities:

 

The Director plays a critical role in the success of the business by planning, directing and supporting all activities of the agency and participating in planning for the agency’s future.

As a member of the Management team, you will work with the team to implement and enforce policies and procedures and ensure compliance with all HCBS Standards. This includes submitting reports, surveys, program evaluations, and other documentation as requested. 

The Director will encourage a positive, supportive approach toward promoting the growth and independence of those served.

The essential duties and responsibilities include, but are not limited to:

  • Plans, directs, coordinates and monitors the delivery of all direct and indirect services to those served by CHAM.
  • Implements policies and strategies to encourage staff retention.
  • Manages program revenues and expenditures to achieve fiscal targets.
  • Uses data to continuously improve program operations, service delivery quality, and program efficiencies.
  • Participates in Referral Team meetings and other committees as requested.
  • Conducts scheduled reviews of records to ensure a person’s continued eligibility and appropriateness for services.
  • Supervises department Program Managers of Supported Community Living.
  • Evaluates the performance of Program Managers and assists them in developing skills and techniques to evaluate performance of their direct reports.
  • Ensures the staff are trained in and comply with company policies and procedures.
  • Maintains positive, ongoing relationship with stakeholders.
  • review and approve incident reports
  • Review and approve Individual Service Plan updates
  • Attends annual Person Centered Service Plan meetings as needed.
  • Other duties as assigned.

  • Strong organizational, workflow, and problem-solving skills.
  • Understanding of compliance requirements for regulatory, training and documentation purposes
  • Establish and maintain constructive, collaborative, and cooperative working relationships in a team-oriented environment.
  • Self-motivated with ability to prioritize tasks, managing one's own time and the time of others.
  • Ability to identify complex problems and review related information to develop and evaluate options and implement solutions.

  • Must be thorough and accurate when performing tasks, able to prioritize, delegate and adapt and able to maintain confidentiality of all employer, employee and member information.

  • Ability to evaluate problems quickly and provide recommendations for resolution.
  • Ability to quickly adapt to new computer software and assist others on computer usage.

Qualifications

Qualifications:

  • Bachelor’s degree in applicable field and two years of relevant experience, or 5 years of relevant experience, required
  • Previous experience managing staff required
